At the beginning of the school year, your child was given several forms that need to be completed by you, the parent. These forms are very important and need to be returned as soon as possible.
- The Lunch Program form needs to be returned by everyone enrolled in the program for the 2009-2010 school year. This form is now online on the school webpage under the cafeteria program tab.
- The Student Information sheet needs to be completed with accurate phone numbers, addresses, and allowed pick up persons. If any information changes throughout the year, please let the office know. This information is very important if there should be an emergency.
- The Technology Forms need to be signed and returned. If check yes, this allows your child to access Study Island, Success Maker, and many other educational programs used to help students excel. If you checked no, students will not be allowed to use the computers within the district. If you would like to change a no, please email or call your child’s teacher and they will send you another form to complete.
